Health Requirements

Proof of Immunizations

All students are required to show proof of immunizations and adhere to the most current requirements regardless of age.

Students in the Early Childhood Education Pre-School - Pre-K must have the following immunizations prior to the first day of school.

  • 3 POLIO
  • 4 DTP
  • 1 MMR
  • 3 HEPATITIS B
  • 1 VARICELLA

 

Students in K – 8 must have the following immunizations prior to the first day of school.

  • 4 POLIO
  • 5 DTP
  • 2 MMR
  • 3 HEPATITIS B
  • 2 VARICELLA
  • 1 TDAP AFTER AGE 7 FOR 7TH GRADE
 

*Students entering 7th Grade must have documented proof of a Pertussis (Whooping Cough) Booster Immunization (Tdap) given after the age of 7. This is a CALIFORNIA STATE requirement and must be met with documentation turned in prior to the first day of the school year. Due to the passage of SB277, Personal and Religious Beliefs cannot be accepted. 

Report of Health Examination

FIRST GRADE ENTRY: All students moving into 1st grade are required to have a completed Report of Health Examination for School Entry (PM171A) signed by their physician. The law allows for the health examination to be completed up to 18 months before entry into 1st grade or within 90 days thereafter. Copies of the form are available in the front office and by clicking on the link to the right.

Life-Threatening Allergies

Any child with a life-threatening allergy must have an “Allergy Emergency Care Plan” filled out and signed by a physician before the first day of school. If a child requires administration of an EPI-PEN for an allergic reaction, the EPI-PEN must be brought to the front office before the child’s first day of school.
